Thirteen states have laws allowing the use of medical marijuana. How can you get a prescription in Oregon?

You must be a resident of the State of Oregon. You must be able to prove that you are a resident. Proof can include:

Current Oregon photo ID card or,

Current photo and a copy of your Oregon voter’s registration card

You must have a condition that requires the use of medical marijuana.

These are:

Cachexia – the severe loss of weight combined with fatigue, weakness, and loss of appetite.

Cancer

Chronic pain

Glaucoma

HIV or AIDS

Multiple sclerosis

Seizures

Severe nausea

Migraine

Amyotrophic lateral sclerosis

Each state has its own set of conditions, but these are the most widely agreed upon.

Make copies of all documents before you fill anything out.

Go online and obtain the forms. The link is below.

Download the proper forms at: http://oregon.gov/DHS/ph/ommp/forms.shtml and print them out. The forms you will need are:

Application to Register for Participation in Medical Marijuana Act Program and

Attending Practitioner Statement

Read all documents completely before you fill anything out.

Make an appointment with you primary care physician and give them a copy of the: Attending Practitioner Statement.

Have them fill out the form and return it to you. Look the document over. Make sure that all parts are filled out before you leave your doctor’s office.

You must find a physical grow site for your medical marijuana. It must be listed on your application. You may grow your own.

Reread the: Application to Register for Participation in Medical Marijuana Act Program. Fill it out. Recheck it. Leave no blanks. Sign the document.

Make a check or money order payable for $100 to: “OMMP” or “DHS/State of Oregon”. This amount is non-refundable.

If you receive Food Stamp benefits, Supplemental Security Income, or a member of the Oregon Health Plan, you may qualify for a reduced application fee of only $20.

Make copies of all documents.

If you move, you have 10 days to notify the Medical Marijuana Program in writing with your change of address. They will not accept a change of address by telephone.
